Community Care Worker’s Required

This role supports older adult patients living in the community who need support with daily living after they are discharged from hospital. This supports people with long‑term and short-term health conditions, or those recovering from illness by delivering person-centred care in their home.

 

Current requirements for Carer’s: Living Care, Respite Care, and Home Care experience.

 

Location: Leeds, Shipley, Gosforth, Castleford, Wakefield, Batley, Bradford, and Huddersfield.

 

The role focuses on promoting independence, dignity, and quality of life, while ensuring safety and well-being. The carer will provide long-term and short-term person-centred support to individuals living at home, or who have recently been discharged from hospital and require daily or ongoing care/living support. The role also focuses on rehabilitation, stabilising daily routines, preventing readmission, promoting independence, and ensuring the individual can safely manage at home while recovering from illness, surgery, or a medical event. Skills Needed:

Level 2 or 3 in Health and Social Care

Min 6-12 months’ experience

Full UK driving licence is essential

Full training provided

Self-employed also welcome

Morning, Afternoon, Evening

Part-Time
