Community Care Team has been delivering high-quality care since 1992, supporting people across Preston, Chorley, South Ribble, and surrounding areas including Leyland, Penwortham, Walton-le-Dale, Lostock Hall, Bamber Bridge, Euxton, Whittle-le-Woods, Clayton-le-Woods, Astley Village, and Buckshaw Village.

Our main aim is to improve the quality of life for individuals who choose to remain living safely and comfortably in their own homes. We provide a flexible, person-centred service tailored to individual needs, whether funded through Social Services, respite care, personal budgets, direct payments, or privately arranged support.

We have a dedicated team of experienced and dynamic staff who are committed to delivering reliable, respectful, and consistent care. All team members are fully trained, appropriately qualified, and carefully selected to ensure they can integrate seamlessly into each service user’s home environment without disruption.

Our services include support with personal care, safe transfers, meal preparation, medication assistance, light housekeeping, companionship, pop-in visits, respite care, and 24-hour support where required. We work closely in partnership with families, carers, and other professionals to ensure coordinated, high-quality care in line with all relevant policies and procedures.

We are proud of our low staff turnover, which reflects the strong relationships and supportive culture within our organisation. Our Registered Manager has been with Community Care Team for 24 years, and several of our carers have been with us for 9 to 15 years, demonstrating our commitment to continuity, reliability, and long-term care relationships.
